i am just wondering, which carriers actually utilizes IMEI blacklists?

Non of them do - they would block SIM cards instead.
To block IMEI requires additional network equipment and would cost the carriers a lot of money for no reason.

i have heard that T-mobile may use blacklisting in USA (see howardforums.com).

other than tmobile. only cdma providers do.

For cdma is easy to have blacklists cuz u can't use one's handset in other's network.
