I don't know whats going on now. Every time I shut down my phone and turn it on again all the shortcuts in the ''quick access'' menu (the menu that comes up when you press the button next to the call answer button) are gone. This includes: bluetooth, track id, main menu, and my 2 own shortcuts: opera mini 4.2 and alse 5 beta. After I have restarted the phone there is only 1 shortcut: It is a block (like when your phone doesn't understand a font, one of those blocks that the letters gets replaced with), and when I press it nothing happens, the menu just closes.
I have R1FA035 GENERIC firmware installed, a modded camdriver, modded font (one of diezel's), modded acoustics, flash menu, NO patches(my phone is still in retail form)

Does anybody know what can cause this?

i've experience missing shortcuts too in the quick access menu. that happened when i try to launch a jar software and suddenly the phone hang. i removed the battery, restarted the phone and only to find out the shortcuts are missing.

i switched off the phone, this time i took out both the battery and the memory card, placed them back and restarted the phone.......lucky me the shortcuts are back again.

in my case, i believed this is a compatibility issue with one of the jar file, though i can't explain the rational

UPDATE: I tried shutting down the phone and taking out the battery and mem card and switching on again but the shortcuts were still missing. I manually put back the shortcuts but when I restarted they were gone again.

What is going on?

You might now need to try a master reset, this can be accessed in Settings > General tab > Master reset (at bottom of list) > Reset all, this deletes all setting and user data e.g. contacts (you can copy back these up to the sim card or memory card) notes, dates in calendar, messages, pictures etc. that’s on the phone memory. I don't think it deletes stuff off the memory card but when I do a reset I always take it out as I'm like that... so copy the stuff on to the memory card and take it out...

That should hopefully do the trick for you, if not your next step would be to reinstall the firmware using SEUS.

this can happen due to corrupt FW (for whatever reasons) and you should try as what adsada suggested, just remember to backup your stuff pal...
