Each phone has a unique IMEI number which is encoded in the phone, and broadcast to the networks when it's on and connected. When you report a phone lost or stolen, most networks will then bar the IMEI of your phone, meaning your phone cannot be used to connect to any network, regardless of what SIM is used. I believe this is fairly international now, subject to agreements between the various networks.
It is possible to get around this on some handsets by changing the IMEI, but it's not very easy and in many countries it's actually a criminal offence to attempt it.
