T-Mobile, Cingular and AT&T are the three US GSM carriers. If you're going prepaid, I think T-Mobile and Cingular are your only choice (correct me if I'm wrong). With T-Mobile to get a prepaid sim it was as easy as walking into the store, paying $50 ($20 sim charge + $30 airtime ~= 125mins or something), inserting the sim in my phone seconds later getting an SMS with my phone #. The only think they asked at the store was my name.
