Do I stick in UK, Great Britain or England?
 I don't wanna go and get another form.

Well I put scottish on mine 
I am only british if my mother or father is born outwith Scotland but they aint so am Scottish.
My gf puts brittish cause her dad is welsh so she is half welsh.

why don't u fill it out online... then they send u the printed form... that way u can practise spelling your name correctly!
