manual focus at zoom 4.0 is minimum possible distance from camera to object where camera can do sharp picture

this kind of effect is possible only in 2 situations:
1. no autofocus but manual focus (~3.0) is used
2. macro mode is enabled <--- this is most probably in your case

when macro mode is enabled then macro range scan is only used because it's faster than full range scan (it's useless in close shots)
