I had a quick search of the forum, but didnt come across anything.
I was just wondering if it would be possible to introduce Private Message Receipts.
So you could effectively track whether the PM has been read or not. (the reviving user would still have the option to send/ not send the receipt back)

Well, this is quite an old version of PHPBB.
Search for : PHPBB, mod, pop up box, notification, private message.
In phpbb2, the code used for Minted Forums (sigh...) there was an automatic pop up box notification built in. Laffen could look at the code, all the things im getting when searching is PHPBB2.0.10 information... not good.

If you wanted a real time pop-up notification, you'd have to use a Java applet. PHP could be used to churn out DHTML to produce an animated pop-up notification, but it would only appear when you refresh the page or click to a new page within Esato.
You could use a javascript to refresh the page automatically every few seconds. I don't know about you, but that would seriously get on my tits
EDIT: Or possibly an ActiveX control but that would only really work in IE.
EDIT Again: Or flash or shockwave I suppose. Whichever way, I don't think you can do it natively in PHP
