my k810i problem with memory card

hi everybody i need help, ive had my se k810i now since august last year and recently my phone decided it wasnt going to accept my san disk m2 128mb memory card! i took it out and put it back in turned my phone off and on and it comes up the usual warning! content on memory stick will be lost format anyway? so i press yes and it askes me for the phone lock code well i dont have it ive tried doing 0000 but it doesnt work it comes up formatting failed can anyone help?
