Want to send a birthday card to Bremen, Germany. And need some translation help!

well a friend of mine is in Bremen
and i want to send her a birthday card...

i have got her street address, house no, postal code and place

the order will be like:

To,
*her name*,
*street address* , *house number*,
*postal code*, then the *place*(she is in Bremen by the way)


anything else i should write/add in the address?

and how to write "happy birthday" in german lingo?
is it "alles Gute zum Geburtstag" or "glucklicher geburstag"?

German addresses are abit like Brit postal addresses

put "Deutschland" on it eg Germany !
