Setting ps2 slim online using blueyonder 1meg

I can't seem to set up the settings for getting my ps2 slimline online via my blueyonder 1meg interent

you need to plug the modem into the slim ps2 and put the network access disc into the ps2, i think you need to buy the disc separately though
